I have one that I made for a B31.3 job I worked on for tracking welds made by pipe spool #, Welder ID etc. No RT but it could be added. There may be a commercial package available that does this already.

If you don't find what you want I can make the above but it may take awhile depending on how lazy I get. I can also make someting that doesn't require MS-Access.
There is a MS Access database I used for inspections of ductwork fabrication that is pretty good for just keeping a general record of inspections.
