Can anyone help explain why/how TABLE 4.3- Performance Qualification - requires root and face bends for thicknesses 3/8 to 3/4" thick , and does not have an alternative to do side bends until 3/4 " and over ?

I thought it might have been a mistake in the 1999 edition, but I now see the same in the 2007.

Procedure Qual does allow side bends over 3/8 (Table 4.2)

Does anyone out there actually do root and face bends on up to 3/4 thickness ?

I know I can calulate the die diameter required , but that puts you up to close to a (2 inch radius) 4 inch diameter bend diameter (Figure 4.9 Nomogram @ 20% elong.) !

If there is an errata, could it not have been corrected after 8 years ?

Although it seems to require face and root bends for greater than 3/8" and less than 3/4", it doesn't have to be over 3/8" thick when bent.  See figure 4.17.  If T is thicker than 3/8", the opposite side of the bend is machined off until the overall specimen thickness is 3/8".

This may help your question.
Look to the left of the table " Type and number of test required" It has 2 foot notes read foot note #2 for 5g and 6g for pipe.but again this is probably referring to thickness exceeding 3/4" its not real clear.
If your asking about plate I dont know if the code mentions side bends for 3/4" and less.
