     I have seen it in coil stock only .004" thick, didn't show up on either side, but split wide open when formed. This comes from a slag inclusion that gets rolled out and can run a great distance in the material.

I agree with Dave on slag inclusions.  The story I got from one of my customers is that the problem can start with a cast ingot.  Apparently, once the ingot has cooled, the top portion that includes a lot of slag is removed before the good stuff is rolled.  If the "bad" stuff is not removed completely, the slag gets rolled and can cause delamination.
