I will RAMFO for that sh** but that seems to be an unpopular standard and procedure in our "git-r-dun" attitude and frame of mind anymore.

I remember hearing that safety etc goes in a complacency etc cycle. After a major accident safety is ramped up, however after 15+ years of getting by without an accident people get relaxed and start cutting corners etc. That's when a major accident happens again. It's preventable but it's human nature
