i have had mine for a while. it is 132.50. money order or credit card. as of april 2009 it will be required to enter any plant that has a port that is regulated by the coast gaurd. It is an exstensive background check done by the goverment, it is good for five years then you will have to renew. you can go on the website to set up an appointment that is the best way to go it is quicker. what ever place you go to your appointment is the place you have to pick the card up at in 3 to 5 weeks. there is no class you have to take, but you do need 2 forms of id such as birth certificate and state issued id.
I heard of some 8 hour waits at the office but no class. I got lucky and was in and out both trips but some people around here weren't so fortunate.
