I have been using databases for years for managaement of welding and related information. I have a very simple access .mdb file available for download at http://www.weldinginspectionsvcs.com/welderqualdownlaod.htm

This database requires microsoft access 97 or later and allows the user to select a welder from a list, select a welder test type from a list, and then enter/edit the Guided Bend or RT results, then print the form.

This application is not compiled so all source code and access objects can be manipulated as needed.

The user should be familar with the use of databases but if you have any questions you can post them on the forum on my site.
