Is there a code or guideline on what the maximum branch size is off a header can be, I have always heard 1/2 the size of the header or less is acceptable, so that means we couldn't do a 10" on a 12",
Any input on this subject would be appreciated.

Not that I am aware of.  I have seen size-on-size saddles in boiler headers.  The size of the branch opening, wall thickness of the run pipe and bending loads from thermal expansion determine whether the pipe can be saddled on without additional reinforcement or if it requires additional reinforcement from weld metal, olet fitting or saddle reinforcement.
