What is the difference between a WPS, Qualified by testing and a PQR?  This may be so obvious that I keep overlooking it.

There is a big diference.
A WPS is qualified in some way or another, your PQR is a document on where you record certain variables during your qualification test.

There is a lot of different ways to qualify a procedure, it depends on the applicable code you are qualifying according to :)

WPS = Welding Procedure Specification
PQR = (welding) Procedure Qualification Record

a "WPS 'qualified by testing'" has a Procedure Qualification Record (PQR) to show actual values used in qualification, welder who did the qualifying weld, and test results.  SO... a WPS 'qualified by testing requires 2 pieces of paper... the WPS, and the PQR
