Was in a find of some handbook / guide as there are some publications from AWS for example on welding inspection.

Ashfaq,

They also have 'The Professional's Advisor on Procedure Qualification' which I have found pretty useful. Has several sample forms for various codes (blank and with sample completions), and other cross reference tables between D1.1, D1.5, B2.1, ASME & API.

Add that to John's suggestion of B2.1 and you will have a very good reference.

AWS B2.1 lays out the process in a pretty clear manner.
