CWI Yearly Eye Exam Form

Where do I find the form that needs to be filled out for the CWI yearly eye exam.
My past employers always took care of this. I work for a small place now and I will
need to do this on my own.
PS..... I am using Pro-Write software to write PQR and WPS's, it works Great!!!

We had a customer audit recently, they wanted to see a recent eye exam.
Is the visual acuity required for the 9 yr recert? I am sending that in soon
anyway.

Every three years per QC1.

Al is correct, 3 years for QC1 or working to AWS D1.1 as an inspector.
However many employers have their QC manual written to require Acuity Eye Exams annually for the inspectors. Also if your employer does any NDT, ASNT SNT-TC-1A says that the employer shall have a written practice, and many times the NDT Level III will draft the written practice to say that annual eye exams will be on file for all inspectors.
