when we qualify 2 welders (gtaw+smaw) there is a tungsten inclusion in RT Film , if the smaw welder qualify or not, kindly mention it in ASME IX standard

The coupon either passes or it fails. If two different processes or two different filler metals with different F numbers, more than one position is welded, or if two people weld the same (single) coupon, it either passes or it fails.

If a pipe coupon is welded in the 6G position (axis inclined at 45 degrees), one welder welds the root bead using GTAW using a F6 filler metal, and a second welder welds the intermediate beads using FCAW with a F6 filler metal, and a third welder welds the cover layer using SMAW with a F4 electrode, all three individuals are qualified for the respective welding process they used, with the filler metal they used, in the positions they welded, and for the base metal they welded. If one of the three welders deposited unacceptable weld in the overhead or vertical position, all three welders fail.

If two people weld the same coupon one passes and the other fails . All two fails ? And what is my reference to fail them ?

If the coupon fails, both welders fail. You will find thin in Article II of Section IX.
