If you are indicating that you need a blank form for weld inspection, there are blank form examples in the back of AWS D1.1.

If you are indicating that you are looking for filled out examples, break out your Level II training material. You may strike paydirt.

AWS shearwave weld inspection reporting is best completed at the same time as the inspection because of the way that the indications are rated (field copy, clean copy to follow later).

However, if there were no indications to rate, your company may have a form that records all the mandatory info and all you have to do is identify what you inspected and state that no revelant indications were found during the inspection.
