With all due respect,if a welded joint is repaired more than two times,is it necessary to be cut from(B/M & W/M &HAZ) and then perform welding again ?Why ?Is there any code of standard about this topic?

I don't know of any codes surrounding this subject, but I think it is good practice on integral parts. Often in the heavy equipment field, I have seen where a welder has welded over a problem area four or more times, and the matal has turned to cottage cheese. If they had just cut it out and prepped the area right the first time, they wouldn't have had to re-do it time 2, 3, 4, etc.
