I know that the max reinf is 1/8" [per D1.1 para 5.24.4 and Fig 5.4 (D)], and that there is an "unacceptable" underfill.  Is there a min reinf for butt joints? 

For unequal plates, can the reinf = "0" , i.e. be "staight" between the two plates?

There is no minimum reinforcement; it can be completely flush.  I don't see how you'd get there without underfill unless you welded with *some* reinforcement and then ground it flush, though.

The way I read 2.7.1 and 2.16.1.1, yes, it can be straight between the plates as long as the slope is no more than 1:2.5.

That is what happened - they ground it flush afterwards.
