Does anyone have a copy of a continuity log, as I am too lazy to come up with my own.

It doesn't need to be fancy...just documentation that the welding qualification is being maintained and current.

I simply sign the back of the welder's qualification with the date and the name of the project.

That is what I used to do, but after about 7-8 years of maintaining it, it is tattered and yellowed.
I keep everything on electronic format now.

For welder's continuity log, is it sufficient just to record the welding process (eg. GTAW, SMAW) or should other details like WPS no., materials and qualifying range be included?

I have the original Welders qualification which has that information on it.
On this maintenance report or log it would just be redundant information.
Just My procedure and  ΒΆΒΆ's.

Some logs have additional information on them, and others do not. Section IX only requires continuity verification in the process. Even if you have this information on the continuity log you still have to produce the original qualification papers. Having additional information can and I stress can help you as long as everyone looking at the logs can follow them. If you have a lot of welders qualified in several process it can make things easier.
However I have seen really neat logs that had to have the spreadsheet author to explain everything.
If someone reasonably familiar with continuity logs cannot look at the log and figure it out in less than a minute, you are collecting too much information and it is poorly organized.
