One must remember that the N-7, as all SAMPLE forms from AWS, is an older version that has not been altered at the same times as code essentials have changed. You are totally free to use anyformat and/or modify the form in question. I have made several modifications to various forms, especially if you compare them 100% to the forms as shown from AWS.
They are only SUGGESTED formats.
Always go to the applicable code to see what the variables, essential and non-essential, are.

The Welder, Welding Operator, Or Tack Welder Qualification Test Record form in Annex N, with regard to the references to Table 4.12 Welding Personnel Performance Essential Variable Changes Requiring Requalification have changed in recent years, as did the contents of Table 4.12. There used to be eight items in what eventually became the current Table 4.12. That particular Table was revised in the 2004 edition of D1.1. One item was deleted, leaving seven items. The deleted item was the reference to the filler metal and the gas/flux type. The 2002 D1.1 was the last edition to include, in what was then Table 4.11, Item (3). It stated "To an electrode and shielding medium combination not approved by an AWS A5 document". That was at the time, an essential variable requiring requalification. In subsequent editions of D1.1, beginning with the 2004 D1.1 and including the current 2010 edition of D1.1, there is a reference (Table 4.12) to each of the two items you posted, but they are no longer referenced to a specific item number in Table 4.12, as Item (3) that stated "To an electrode and shielding medium combination not approved by an AWS A5 document" has been deleted. It appears that it is no longer an essential variable, and it's references on the form in Annex N should have been deleted.
